Community Home Support Services (CHSP)

HART’s Community Home Support services are Government subsidised services aimed at assisting older people with their transport and social support needs. 

Access to safe and supported transport is essential as you get older and wish to remain as independent as possible.
HART Services have a solid reputation and trusted relationship with our clients built over 30 years of transporting people within the Northern Rivers Community.

Services include:

orange tick small  Personalised medical transport;
orange tick small  Social and Shopping Buses;
orange tick small  Aboriginal Transport Services;
orange tick small  Visiting Vouchers.

 

Who can access Community Home Support?
Question  Are you 65 years or older, or 50 years or older and identify as an Aboriginal and Torres Strait Islander person, or
       50 years or older and on a low income, homeless or at risk of homelessness;
Question  Are you still living at home;
Question  Are you in need of help at home to continue to live independently.

If you answer ‘yes’ to these questions, then you or your carer could enquire and apply for home support services by contacting the My Aged Care contact centre.

Ring 1800 200 422 to register and remember to list HART Services as your preferred aged care provider.
