Home Care Packages (HCP)
The Australian Government's Home Care Package program helps you live independently in your own home for as long as you can.
If you are eligible for a home care package, a subsidy is paid to an approved home care provider of your choice towards a package of care to meet your individual needs.
The subsidy contributes to the total cost of your services and care delivery. It is expected that you will also contribute to the cost of your care where your personal circumstances allow.
Home Care Packages come in four levels, increasing in value from level 1 through to level 4.
Level 1 - basic care needs
Level 2 - low-level care needs
Level 3 - intermediate care needs
Level 4 - high-level care needs.
Services can include:
Domestic assistance
Meal preparation
Personal care
Garden & yard services Transport
Social support & outings
Shopping
Support with technology
Nursing, allied health & clinical care
HART Services delivers home care packages on a Consumer Directed Care (CDC) basis.
This means we will talk to you about your needs and goals and work with you to develop your individual care plan.
You will have choice and control over what services are delivered, where and when they are delivered and who delivers them to you.
Our Client Care Manager will work with you to develop a care package plan and budget outlining how your package is funded and what services and care needs are included.
We will provide you with monthly income and expense statements so that you know how your budget is tracking and we will ensure that your package continues to meet your needs with ongoing monitoring and formal reviews.
Who is eligible for a home care package?
To find out if you are eligible for a home care package or any other help at home services,
Ring 1800 200 422 to register and remember to list HART Services as your preferred aged care provider.

Once a package is approved you can choose a provider. HART Services is an approved age care provider who will work closely with you to put together a care plan.
